A regional healthcare network with 15 hospitals and 200+ clinics needed to unify patient data across disparate systems to improve care coordination and enable predictive health analytics.

The Challenge

Patient data was siloed across multiple legacy EHR systems, making it impossible to get a unified view of patient history. Data quality issues led to duplicate records, and clinicians lacked the tools to identify at-risk patients proactively.

Our Solution

DHD Tech provided a team of three data engineers and one cloud architect who built a HIPAA-compliant data lakehouse on GCP. The platform ingests data from all EHR systems via HL7 FHIR interfaces, performs entity resolution to deduplicate patient records, and powers a suite of predictive dashboards for clinical staff. A readmission risk model was deployed to flag high-risk patients at discharge.
